Your spouse keeps calling about the claim. The person on the phone says they can only speak to you. VA Form 21-4142 does not fix that. VA Form 21-0845 does — for one named person, and only the information you check.
What it actually is
VA Form 21-0845 (revision May 2026) is the Authorization to Disclose Personal Information to a Third Party. VA.gov’s page (updated July 30, 2026) is one sentence: use it to “share your personal information with a non-VA (third-party) individual or organization.” That is the whole page. The supporting forms hub (updated August 17, 2026) repeats it. The additional forms hub (July 8, 2024) never names 21-0845 at all.
The May 2026 PDF is the list. The split is one sentence: 21-0845 lets VA talk out. 21-4142 lets VA pull in.
- 21-0845 — VA may tell the person or organization you name the claim or benefit facts you check. Status. Rate. Payment history. A benefit letter. What you owe. A change of address or direct deposit as information, if you check that box. It is not a clinic request. It is not a VSO appointment. It is not a signature on your application.
- 21-4142 plus 21-4142a — VA may request private treatment records from the clinics you list. Authorization plus provider list. Both expire 12 months after you sign. That split is already on VA 21-4142 vs 21-4142a: Expire in 12 Months. Filing 0845 instead of 4142 does not start a records pull.
Item 10 is one person or one organization. Not both. Not two people on two forms. If you name an organization, you also name the actual people inside it who may hear your information. The PDF says so in the Item 10–13 instructions. A blank org name with nobody under it is how a call dies at identity check.
You can only have one 21-0845 on file at a time. The instructions page is explicit. A new 0845 replaces the old one. Spouse on Monday and the accountant on Tuesday means the accountant is the only one left.
Item 11 is two boxes, and you check only one: limited information, or any information. Limited is Item 12 — check all that apply (status of a pending claim or appeal, amount owed, current benefit and rate, a benefit payment letter, payment history, change of address or direct deposit, or other). “Any information” is Item 13 — one time only, ongoing until you write to stop, or from the date you sign until a date you write. The form does not let you dump tax transcripts through this paper. The instructions say it cannot be used to disclose federal tax information.
Item 14 is the security question. VA asks it each time your named person calls. City and state your mother was born. High school. First pet. Favorite teacher. Father’s middle name. Pick one they will actually know, and write the answer the same way they will say it.
This form does not appoint a representative. A VSO, attorney, or claims agent still files 21-22 or 21-22a. 0845 is “they can hear.” 21-22 is “they can act.”
This form does not let someone sign your application because you cannot hold a pen. That is VA Form 21-0972, Alternate Signer Certification, submitted with the application it covers. Some other PDFs (including 20-10207) will not accept a third-party signature unless 21-0845 is already of record. That is still not 0972, and it is still not 21-22.
A beneficiary VA has recognized as incompetent cannot execute 21-0845, and VA will not accept one from them. The form says so under the title. After a competency rating, the path is a fiduciary — not a spouse 0845 hoping the call center treats it like one.
If the claimant is not the veteran, Item 6 is that claimant. Item 10 cannot be the same person as Item 6. You are authorizing VA to talk to someone else, not to yourself.
Do this today
- Name one person VA should actually be allowed to talk to. Spouse, adult child, or one organization with the people inside it written on the line. If you need a VSO to run the claim, that is 21-22, not this form.
- Open the May 2026 PDF. Item 11: limited or any. Item 12 or 13: the boxes and the stop date. Item 14: one security question and one answer. Sign Item 15. Date Item 16.
- File online when you can. The live tool is authorize VA to share your information with a third party. Screenshot the confirmation. A saved draft is not on file. The PDF also names Direct Upload at access.va.gov.
- If you must mail: Department of Veterans Affairs, Evidence Intake Center, P.O. Box 4444, Janesville, WI 53547-4444. Paper is the date VA receives it, not the postmark. Certified mail with return receipt is how you prove receipt. Keep a copy. The PDF tells you to.
- If the real job is private clinic records, stop and file the other forms. 21-4142 and 21-4142a. 0845 will not pull a chart.
- If you change your mind: writing, 1-800-827-1000, or Ask VA. VA will stop new disclosure. What they already said under the old permission stays said.

What people get wrong
“I signed 21-4142, so my wife can call about the claim.” 4142 is a clinic pull. The call center is 0845. Different direction. Different form.
“I’ll file 21-0845 so VA will request my private doctor records.” Pull is 4142 plus 4142a. 0845 does not start a request to a hospital.
“I’ll put my spouse on one 0845 and my accountant on another.” One on file. The second one replaces the first.
“I’ll list the whole law firm and let whoever answers the phone ask.” Organization requires named individuals. The security question is asked every call.
“0845 means they can change my direct deposit.” If you check that box, VA may tell them about a change of address or direct deposit. Telling is not acting. Acting on the claim is 21-22 or 21-22a.
“She handles my mail, so she can sign the 21-526EZ.” Signing an application because you cannot is 21-0972, attached to that application. 0845 is talk, not a signature.
“VA rated me incompetent, so my spouse files 0845.” The form will not take that signature. Fiduciary is the path. 0845 from an incompetent beneficiary is a rejected paper.
“I checked any information and left Item 13 blank.” Any-information needs a term: one time, ongoing until you write to stop, or a stop date.
“The VSO has 21-22, so my spouse is covered.” The VSO is covered. Your spouse is not, unless you name them on 0845.
One named person. The boxes you check. Not a records pull.
